7/29/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, QMHP-C engaged her in a discussion to determine any new behaviors within the home. Mother reports that she is really tired of her getting up in the middle of the night cussing as though she donât have any sense. Ashiyah reported to QMHP-C, âWhat choice do you have when you have to sleep on the fucking floorâ. Mother reports that she chooses to sleep on the floor because she can go and sleep with her brother if she did not want to be sleeping on the floor. Ashiyah reports that she is not sleeping with her brother at her age and that is why she is always arguing in the home. 2. Behavior: Ashiyah actively participated in the session but needed some redirection as she was getting upset with her mother when her mother was discussing with QMHP- C on how she (the mother) tells her the same thing all the time about her behaviors. 1.Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C engaged Ashiyah in role-play scenarios to assist Ashiyah with identifying anger management skills and problem-solving solutions. QMHP-C provided Ashiyah with information on anger and the definition. QMHP-C explained to Ashiyah that the purpose of this activity was to support Ashiyah with explaining the ramifications of anger. QMHP-C engaged Ashiyah in respectful communication rather than interacting negatively with others. 2. Intervention: QMHP-C illustrated how to manage self-controlling behavior when frustrated and feeling upset. QMHP-C explained to Ashiyah that she needs to communicate with her mother more when she is feeling anger or things are bothering her to keep from issues arising that have in the past. QMHP-C encouraged Ashiyah to identify how she believes her impulses encourages negative decision-making. QMHP-C modeled effective ways Ashiyah could demonstrate communication and assertiveness. 3. Intervention: QMHP-C pointed out wrong doings and multiple opportunities to avoid the conflict. QMHP-C processed the incident while encouraging Ashiyah to accept responsibility for her actions and the role she played in the conflict. QMHP-C role played alternative ways to handle similar situations in the future. 1.Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. During the session (initial counseling session) it appears that Ashiyah struggles with controlling her anger and frustrations evident by talking back and stomping her feet when upset (as described by her mother). 2.Response: Ashiyah demonstrated an understanding of anger management skills and problem solving however she struggles with implementing those skills when needed. Ashiyah discussed with QMHP-C that she has to get a feel for QMHP-C before she began just opening up to QMHP-C about situations that has happened in her life. 3. Response: Ashiyah was able to process positive ways to handle conflicts, however she is unable to demonstrate them behaviors while angered. She actively participated in role playing with QMHP-C.
